Required Student Enrollment Forms

Below, you will find a list off required Enrollment Forms.  The “required” forms are due annually, regardless of the child’s previous enrollment status.  These forms are due when the student first commences their enrollment and at the beginning of each subsequent school year, unless a more frequent submission schedule has been outlined.  A student’s file is considered ‘incomplete’ without all of the required forms on file and this status may impact their enrollment status.

As of this time, the only form that may be due more regularly will be the General Health Appraisal, which is due in accordance with the AAP recommended Well Child Visit schedule.

Records may be provided to the school in any of the following ways:

  • Handed into the Campus Business Office
  • Mailed to the Enrollment Team
  • Faxed to (720.545.9739)
  • Emailed to the Enrollment Team (records@midtownmontessori.org)
